Who Can Apply?
Only one parent, who is a citizen of Ukraine, needs to submit the application. The child and the other parent (whether a foreigner or a Ukrainian citizen) do not need to be present.
Required Documents
To register your child’s Ukrainian citizenship by birth, the following documents are needed:

Application Form: A completed application for registering the child’s acquisition of Ukrainian citizenship by birth.

Proof of Ukrainian Citizenship: The original and a copy of the Ukrainian parent’s passport or another document (as per Article 5 of the Law of Ukraine “On Citizenship of Ukraine”) showing that one parent was a Ukrainian citizen at the time of the child’s birth.

Child’s Birth Certificate: The original and a copy of the child’s birth certificate issued by the Civil Registry Office of the country where the child was born.
Important Notes About the Birth Certificate

The birth certificate must include the names of both parents.

It must be legalized in the country where the child was born.

The certificate and the legalization stamp must be translated into Ukrainian.

The spelling of the child’s name and surname must follow transliteration rules (e.g., “Michele” becomes “Микеле”, not “Михайло”).

The child’s surname must match the surname in the Ukrainian parent’s passport for traveling abroad.

The parents’ names and surnames in the birth certificate and its translation must also match those in their Ukrainian passports.
Surname Assignment
The Civil Registry Office will assign the child either:

The father’s surname, or
-
The mother’s surname if the father is not listed on the birth certificate.
Decision Process

The head of the Ukrainian Embassy (or their deputy) will decide on the application within one month of receiving the documents.

If the application is approved, the child is registered as a Ukrainian citizen and receives a Certificate of Registration as a Citizen of Ukraine.

If the application is denied, the applicant will be notified in writing within one week of the decision.
About the Certificate

The Certificate of Registration as a Citizen of Ukraine is issued only once and in one copy.

This certificate is needed to apply for documents like a Ukrainian passport for traveling abroad.

The first issuance of the certificate is free of charge.

Re-registration of a child as a Ukrainian citizen by birth is not allowed.
Lost or Damaged Certificate
If the original certificate is lost, damaged, or taken, the Ukrainian Embassy can issue a replacement certificate confirming the child’s prior registration. The consular fees for this are:

30 USD (issued within 5 business days), or
-
60 USD (issued within 1 business day).
