Name of parents of adoptee:

            (father of adoptee)

            _________Khatoon wife of Md. ____________peer.

            (mother of the adoptee)

Name of Adoptee: _________Fatima aged 15 days.

All residents of village Nanpur,

P.S.__________, District Sitamarhi (Bihar) at

present __________Vihar Colony, Meharban,

Basti Jodhewal, Ludhiana

 Whereas, the adopters for their spiritual benefit, for the benefit of their ancestors and for perpetuation of our all immovable and movable properties and name Benazir ______aged 15 days (adoptee)daughter of Md. __________Peer resident of village __________, P.S.Nanpur, District Sitamarhi (Bihar) at present ___________Vihar Colony, Meharban, Basti Jodhewal, Ludhiana after giving the matter full and due consideration and while in possession of my senses, sound mind and full faculties and before a gathering of relations and friends, do perform all the customary and religious ceremonies and have given by Benazir ________aged 15 days daughter of Md. ________Peer resident of village _________, P.S.Nanpur, District Sitamarhi (Bihar) at present __________Vihar Colony, Meharban, Basti Jodhewal, Ludhiana whom the adopters accepted and adopted as a daughter like their daughter.

    The aforesaid Benazir Fatima shall have all the legal rights as their daughter in their family. Both the adopters hereby bind themselves to following terms and condition:-

5. that the adopters have no blood relation with the adoptee and now the said Md. ________Fatima (adoptee) aged 15 days daughter of Md. __________Peer resident of village ___________, P.S.Nanpur, District Sitamarhi (Bihar) at present Harkishan Vihar Colony, Meharban, Basti Jodhewal, Ludhiana and there is difference between them.
6. That the adopters and the adoptee are of the same religion i.e. both are Muslim.
7. Neither of the parties has ever been involved in any Adoption Deed except this adoption deed.
8. That father and mother of the adoptee i.e. Md. ______Peer and ________khatoon as mentioned hereinabove waives off their claim on the adoptee and shall not claim in future on the adoptee in any manner as they have handed over their daughter/adoptee to the adopters with their consent and without any ulterior coercion and pressure.
9. That if in future the adopters bear any issue, the same shall be in the like right and entitle in all the movable and immovable property of the adopters in equal shares.
